Collecting Student from Tombolo Academy

Please complete the form below to give consent for someone (other than the student’s parents/guardian) to collect the student from school on your behalf. You may update this information at any time.

If the student is not collected from school at the end of the school day and parents/guardians cannot be contacted, Tombolo Academy will contact an Authorised Nominee to collect your child from school.

If your child has an accident, injury, trauma or illness and the parents/guardian cannot be contacted, you can give consent for an Authorised Nominee to collect your child from school and/or consent to medical treatment. In an emergency, your consent allows the school to seek treatment if necessary.
