Local Excursions Consent

From time-to-time, Tombolo students and staff may visit local venues for the purpose of integrating and enhancing learning outside of the classroom. Please give consent for your child to attend the following local venues:

“At TwistED Science we thrive on curiosity, how things work, how they move and feel. Through interaction with our all-ages science centres, hands-on parties, and curriculum-based workshops, everyone can explore all the fun of science through play.”

I would like to give consent for my child to attend the local excursions listed above. In the event my child’s behaviour is unsafe during the excursion, I understand that it is my responsibility to collect my child from the excursion when contacted by school staff.

Consent to medical attention:

In the event my child requires medical attention during the excursion, where the teacher-in-charge is unable to contact me, or where it is impracticable to contact me, I authorise the teacher-incharge to:

1. Consent to any medical or surgical attention deemed necessary by a medical practitioner.

2. Administer such first aid as the teacher-in-charge judges to be reasonably necessary.

I agree to meet any medical expenses and/or transport costs incurred in the event of sickness or injury.
